The hCG Weight Loss Protocol
Cost: $850 (PRICING TO INCREASE Jan 2012)
Includes: First One Hour Intake Evaluation with Nurse
Initial appointment with the Physician week 2 or 3
Initial blood chemistry analysis if needed
One Urinalysis check as needed
Weekly scheduled weigh-ins (you may visit the clinic as many times as you would like for weigh-ins and vital sign checks. No appointment necessary)
All telephone consultations over the course of treatment.
39 days hCG medication.
Additional food supplement may be recommended.

One final review appointment with the Physician
Not included in cost: Medications other than hCG, syringes, appetite suppressant IF NEEDED, food supplements, or lab work. Additional B-12 injections.

The Out of Town hCG Weight Loss Protocol
Cost: $695
If you are at a distance greater than 200 miles, a complimentary initial telephone consultation will be done to determine whether you are eligible for care.
Maintenance Phase
Cost: $350 per 6 months (PRICE TO INCREASE 2012)
Includes: Unlimited clinic visits for weigh-ins and vitals check.
1 blood chemistry analysis and 2 urine analysis checks if needed
2 telephone consultations with physician
2 telephone consultations with nurse
All in office physician visits will be $60
Monthly B12 injections as needed.

Wellness Consultation
Cost: $750 (PRICE TO INCREASE 2012)
Includes: Complete Comprehensive Initial History and Physical Exam Appointment with the Physician. This Comprehensive Exam is the same evaluation the President of the
Complete metabolic functional panel evaluation to include hormone evaluations.
Review, evaluation, and recommendations are made of all medications and supplements to ensure you are only taking things that are worthwhile.
Diagnostic B12 injection.
Recommendations for weight loss and diet as needed.
If appropriate samples of nutritional supplements.
A written summary of evaluation and Wellness Plan.
Two 45 minute follow-up appointments to discuss the written evaluation summary and follow-up on the Wellness Plan.
Three 15 minute Physician Telephone Consultations over a 3 month period for Wellness Plan follow up
No appointments necessary for all weigh–ins or vital sign checks, however appointments are recommend.
Not included in cost: Medications, supplements, or lab work.
All regular 45 minute Physician Consultations beyond what is included are $95 and all Telephone Consultations with the physician beyond what is included are $25.
Concierge Medicine
Cost: $1,800 Monthly and Quarterly Payments Available (PRORATED BASED ON JULY 1st START DATE)

Procedures such as a laceration repair, putting a dislocated joint in place or any other invasive procedure have an additional cost. That cost is dependant on the procedure and you will know the cost before the procedure is started.
Weekend appointments additional charge.
House calls $495.
